The unified theory of acceptance and use of technology (UT-AUT) is a technology acceptance model.

The questionnaire is composed by the following constructors: 1) performance expectancy, 2) effort expectancy, 3) social influence, and 4) facilitating conditions. The first three are direct determinants of usage intention and behavior, and the fourth is a direct determinant of user behavior.

2.1 Categorical fields:

Categorical fields included in this dataset are ordinally encoded and represents the degree from 1 to 7 with agreement with the statement indicated in the field (the value 0 is given when no answer was provided):

Effort Expectancy:
EE1: My interaction with the IoT device is clear and understandable
EE2: It is easy for me to become skillful at using the IoT device
EE3: I find the IoT device easy to use
EE4: Learning to operate the IoT device is easy for me

Performance Expectancy:
PE1: I find the IoT device useful for my purposes (daily living/job)
PE2: Using the IoT device enables me to accomplish tasks more quickly
PE3: Using the IoT device increases my productivity
PE4: If I use the IoT device, I will increase my chances of getting a better quality of life

Attitude Towards Using Technology:
AT1: Using the IoT device is a good idea
AT2: The IoT device makes study more interesting
AT3: Working with the IoT device is fun
AT4: I like working/using with the IoT device

Social influence:
SI1: People who influence my behavior think that I should use the IoT device
SI2: People who are important to me think that I should use the IoT device
SI3: Professors in my classes have been helpful in the use of the IoT device
SI4: In general, the doctor/general practitioner has supported the use of the IoT device

Facilitating Conditions:
FC1: I have the resources necessary to use the IoT device
FC2: I have the knowledge necessary to use the IoT device
FC3: The IoT device is not compatible with other systems I use
FC4: A specific person (or group) is available for assistance with the IoT device difficulties

Self Efficacy:
SE1: I can complete a job or task using the IoT device, if there is no one around to tell me what to do as I go
SE2: I can complete a job or task using the IoT device, if I can call someone for help if I get stuck
SE3: I can complete a job or task using the IoT device, if I have a lot of time to complete the job for which the software is provided
SE4: I can complete a job or task using the IoT device, if I have just the built-in help facility for assistance

Anxiety:
AX1: I feel apprehensive about using the IoT device
AX2: It scares me to think that I could lose a lot of information using the IoT device by hitting the wrong key
AX3: I hesitate to use the IoT device for fear of making mistakes I cannot correct
AX4: The IoT device is somewhat intimidating to me

Behavioral intention:
BI1: I intend to use the IoT device in the next semesters
BI2: I predict I would use the IoT device in the next semesters
BI3: I plan to use the IoT device in the next semesters

The EQ-5D-3L is a standard questionnaire of QoL self-assessment provided by euroqol.org.

The EQ-5D-3L descriptive system comprises the following five dimensions: mobility, self-care, usual activities, pain/discomfort and anxiety/depression. Each dimension has 3 levels: no problems, some problems, and extreme problems. The patient is asked to indicate his/her health state by ticking the box next to the most appropriate statement in each of the five dimensions.
